How to book an appointment
If you would like some support you can go to the 'How to refer' to refer yourself or someone else immediately.
Once you have completed the referral form (don't worry it's pretty short) we will take a little bit of time to process your information and then one of our staff team will give you a call to arrange a telephone assessment.
We work pretty quickly to make sure you receive the best possible service form us. You can expect a call from us in less than five working days (we aim to call you back as soon as we can) from submitting the referral form. If it is not convenient for you when we call, we will arrange a suitable time to call you back.
When we call you, you will have the opportunity to ask any questions you may have and we will ask you for some more information about why you need to access support with us.
What happens after the telephone assessment?
Once you’ve completed the initial telephone assessment we’ll book you in to see one of our support workers.
